HIPAA Notice of Privacy Practices
This notice describes how your medical information may be used and disclosed and how you can get access to this information.
Please review this notice carefully. It describes your privacy rights and our legal duties regarding your health information. We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this notice.
How We May Use and Disclose Your PHI
We may use and disclose your PHI for: (1) Treatment — sharing with physicians, specialists, and care team members involved in your care; (2) Payment — processing insurance claims and collecting fees; (3) Health Care Operations — quality assessment, training, accreditation, and business management activities. These uses do not require your authorization.
Uses and Disclosures Requiring Your Authorization
We will obtain your written authorization before using or disclosing your PHI for marketing purposes, for the sale of PHI, or for psychotherapy notes. You may revoke your authorization in writing at any time.
Special Protections for Sensitive Information
Certain categories of PHI receive additional protections under federal and state law, including mental health records, substance abuse treatment records, HIV/AIDS status, and reproductive health information. We will not disclose this information without your specific authorization except as required by law.
Your Rights Regarding Your PHI
You have the right to: (1) Inspect and copy your PHI; (2) Request amendments to your PHI; (3) Request restrictions on uses and disclosures; (4) Request confidential communications; (5) Receive an accounting of disclosures; (6) Receive a paper copy of this notice; (7) File a complaint if you believe your rights have been violated.
Our Duties
We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, abide by the terms of the current notice, and notify you in the event of a breach of your unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with FirstCall's Privacy Officer at privacy@firstcalltelemedicine.com or with the Secretary of the U.S. Department of Health and Human Services at www.hhs.gov/hipaa. You will not be retaliated against for filing a complaint.
Effective Date and Changes
This notice is effective January 1, 2026. We reserve the right to change this notice and to make the new notice effective for all PHI we maintain. The current notice will always be available on our website.
