The Medication

What is the Wegovy® pill?

Wegovy® pill is once-daily oral semaglutide 25mg — the first FDA-approved oral GLP-1 receptor agonist for weight management. FDA approved on December 22, 2025, it contains the same active compound as the Wegovy® injection but in tablet form. It works by mimicking the GLP-1 hormone to suppress appetite, slow gastric emptying, and improve insulin sensitivity.

In the Phase III OASIS 4 trial, participants taking oral semaglutide 25mg once daily lost an average of 16.6% of body weight at 64 weeks with full adherence — comparable to the injectable form. One in three participants achieved 20% or greater weight loss. The pill does not require refrigeration, making it more convenient for travel and daily use.

Pill vs. injection: Both Wegovy® forms contain semaglutide and deliver comparable weight loss. The pill is taken once daily and requires no refrigeration. The injection is taken once weekly. The key pill requirement: take on an empty stomach with up to 4oz of plain water and wait 30 minutes before eating or taking other medications.

Critical Instructions

The 30-minute rule — why it matters

The Wegovy® pill uses an absorption enhancer (SNAC) to enable oral absorption of semaglutide. For this to work correctly, the pill must be taken in a very specific way — and deviating from these instructions significantly reduces effectiveness.

Step 1 — Empty stomach

Take first thing in the morning before eating or drinking anything other than plain water. Stomach must be empty.

Step 2 — Small amount of plain water only

Swallow with up to 4oz (120mL) of plain water only. Do not use other beverages. Do not crush or split the tablet.

Step 3 — Wait 30 minutes (non-negotiable)

Wait at least 30 minutes before eating, drinking anything other than plain water, or taking other oral medications. This window is essential for absorption — eating or drinking early significantly reduces how much semaglutide is absorbed.

Gradual dose escalation allows the body to adjust and minimizes gastrointestinal side effects.

Starting dose 1.5mg

Begin at the lowest dose. Take daily on an empty stomach, wait 30 minutes before eating. Most tolerate this dose well.

Month2
Escalate to 4mg

Dose increased to 4mg. Appetite suppression becomes more noticeable. GI side effects typically diminish as body adjusts.

Months3–5
Escalate to 9mg then 25mg

Gradual dose escalation continues toward the 25mg maintenance dose over several months.

Maintenance at 25mg

Important Safety Information

Wegovy® pill is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Do not use during pregnancy — discontinue at least 2 months before attempting conception. Not recommended during breastfeeding due to the SNAC absorption enhancer. Common side effects include nausea, vomiting, and diarrhea — particularly during dose escalation. Pancreatitis has been reported — seek immediate care for severe abdominal pain. Gallbladder disease risk is increased. Do not take with other semaglutide-containing products. The 30-minute empty stomach rule must be followed for proper absorption.
